We asked her history again: she had received facial whitening treatment at the beauty clinic 6 months ago. The lab tests demonstrated a marked increase in urine mercury and the neuroelectromyogram showed positive. After consultation with the pain unit, neuropathic pain (NPP) was considered due to the more severe LBP and numbness, with occasional muscle fibrillation in the fourth and fifth fingers of her hands. After taking these drugs, both LBP and numbness were not improved, while the drowsiness was obviously improved, and the quality of sleep was still bad. After a series of scale screening, including Symptom Check-List 90 (SCL-90), Self-rating Depression Scale (SDS), and Self-rating Anxiety Scale (SAS), the patient received a diagnosis of somatic symptom disorder (SSD) with mild anxiety, and was treated with mirtazapine and duloxetine for a month. ![]() We then suspected that the pain was related to central sensitization and recommended the Psychosomatology Unit for the patient. There was no improvement in LBP, while numbness and discomfort in both hips were complained of gradually. She was treated with diclofenac sodium and ADA for a month. It was possible that the atypical inflammatory LBP was caused by SPA activities. MRI scan revealed the presence of bone marrow fat infiltration in both SIJs ( Fig. 1). In 2018, the patient presented with 2 month history of intermittent soreness pain and sleep disorder. Six months later, she was in remission and bone marrow fat infiltration replaced the BME on MRI. A diagnosis of spondyloarthritis was made and she responded to celecoxib and ADA. Her ESR and CRP were both raised and she was HLA B27 positive. ![]() Plain X-rays did not show sacroiliitis but the MRI scan revealed the presence of bone marrow oedema (BME) in both sacroiliac joints (SIJs). In 2016, the patient initially presented with a 6-month history of LBP and early morning stiffness. We report the case of a 30-year-old female of SPA who initially responded to NSAIDs and adalmumab (ADA) but later presented with non-inflammatory LBP.
